Phelan-Mcdermid Syndrome Foundation Membership

Please use this Family Membership form if a member of your immediate family is diagnosed with Phelan-McDermid syndrome.

Enter the main household contact FIRST, then enter other family members, including your family member diagnosed with Phelan-McDermid syndrome as sub members.

A household includes people living at the same address and PMS individuals who live at a different address (group home or school).
